Chat forms
Chatbot forms that appear in the conversation when they fit
Build short forms on the Actions tab, such as a quote request, a return or a booking enquiry. Your AI agent shows the right one under its reply when the conversation calls for it, the visitor fills it in, and the answers reach you on the Leads tab, by email, in Slack or through a webhook.
Visitor: Hi, do you print custom T-shirts for events? We need around 80 for a company run in June.
Agent: Yes, we print custom T-shirts for events, with screen printing from 20 pieces. Prices depend on the shirt, the number of print colours and the quantity, so the quickest way to a price is the quote request below. Fill it in and our team sends you an offer.
Visitor: [Fills in the Quote request form: product, quantity 80, print colours 2, needed by 12 June, email, and clicks Send]
Agent: Thanks, your request is with our team. You will get the quote by email.
Asking for details one question at a time wears visitors out
A quote request needs the product, the quantity, a delivery date and an email address. A return needs an order number and a reason. When a chatbot collects that by asking one question per message, the conversation drags on, visitors give half answers, and someone on your team still has to piece the details together from a transcript. A form inside the chat asks everything at once, checks the answers before they are sent and hands you clean, structured data, while the agent keeps answering questions around it.
Why use forms inside your chatbot
The agent decides when a form helps. You decide exactly what it asks.
All the details in one step
Instead of a back-and-forth of separate questions, the visitor sees every field together under the reply, fills them in and clicks the button once. Required fields and email, phone, number and date answers are checked before the form is sent.
Shown at the right moment
For each form you describe in plain words when it helps, for example “when the visitor asks for a price for a custom order”. The agent reads that and shows the form under its reply when the conversation fits, with a short sentence that points to it.
The fields are always yours
The form is built from the version you saved. The AI model only decides when to show it, never what it asks, so labels, choices and required fields stay exactly as you wrote them.
Answers go where you work
Submissions land on the Leads tab with a link to the conversation, can be emailed to your alert recipients, posted to Slack and sent as a form.submitted webhook, and each form can have a webhook of its own.
Kept apart from your leads
A form submission is stored on its own, even if it contains an email address. It doesn’t add a lead or trigger a new-lead alert, so a return request never shows up in your sales pipeline.
How to add a form to your chatbot
Create the form
On the agent’s Actions tab, go to Forms and click Add form. Give it a name, which visitors see as its title, and add up to 20 fields. Each field has a label, a type such as Short text, Email, Date or Dropdown, and a Required checkbox.
Describe when to show it
In When to show it, write up to 1,000 characters on the moments the form helps. Visitors never see this text; the agent reads it to decide. Optionally set your own button text and the message visitors see after sending.
Check the preview and test it
Keep Agent can show this form on, check the Preview and click Add form. Then ask a matching question in the Playground. If the form shows too often or never, describe the moments more precisely and try again.
Decide where submissions go
Turn on email for new leads to have each submission emailed, tick Form submission in the Slack alerts card, subscribe a webhook to Form submitted, or add a webhook just for this form in its editor to send its answers to your CRM or shop system.
Chat form details
Seven field types
Short text (up to 200 characters), Long text (up to 2,000), Email and Phone, both checked before sending, Number (also with a decimal comma), Date with a date picker, and Dropdown with up to 20 choices.
Up to 10 forms per agent
Build a form for each job, such as quotes, returns and booking enquiries. Forms don’t count toward the limit of 25 actions, and the Enabled switch takes a form out of use without deleting it or its submissions.
One form per reply
The agent shows at most one form under a reply and doesn’t ask for the same details in the chat. No form appears under a reply that already shows the lead form, so the visitor is never asked twice.
Sent once per conversation
Each form can be sent once per conversation. After that it shows as sent, also when the visitor reloads the page, so you don’t get duplicate requests from the same chat.
Submissions and CSV export
The Form submissions card on the Leads tab lists each form’s submissions, newest first, with the answers and a link to the conversation. Export CSV downloads up to 10,000 of them, with a column per field.
A webhook for each form
Give a form its own HTTPS webhook to send, for example, quote requests to your CRM and returns to your shop system. It uses the same signed form.submitted payload, retries and delivery log as the agent’s webhooks.
Everywhere the agent answers
Forms appear in the chat bubble, the inline iframe, the direct link, the Help Page and the Playground. They are saved with the reply, so they also show in transcripts on the Conversations tab.
Your words, their language
Visitors see your form name, labels and choices exactly as you write them. Hints such as “(optional)”, error messages and the default button and thank-you follow the widget language you set.
Frequently asked questions
What is a chatbot form?
A chatbot form is a short form shown inside the chat conversation instead of on a separate page. In intoCHAT you build it on the Actions tab with up to 20 fields, and the AI agent shows it under its reply when the conversation fits, for example when a visitor asks for a quote or wants to return an order.
How is a chat form different from lead capture?
The lead form collects contact details, such as a name, an email address or a phone number, and adds the visitor to your leads. Chat forms collect structured details for a specific request, such as a quote, a return or a booking enquiry. Their submissions are stored separately and never add a lead or send a new-lead alert. You can use both on the same agent.
Can the AI change the questions in my form?
No. The form is always built from the version you saved. The AI model decides only when to show it, based on your When to show it description. It can’t add, remove or reword fields, and it doesn’t ask for the form’s details in the chat instead.
Where do form submissions go?
Every submission is saved on the Leads tab under Form submissions, with a link to the conversation and a CSV export. You can also have submissions emailed to your lead alert recipients, posted to Slack, sent to any webhook subscribed to Form submitted, and sent to a webhook of the form’s own.
Can I send form submissions to my CRM?
Yes, through a webhook. Each form can have its own HTTPS webhook that receives only that form’s form.submitted events, signed with a secret, with retries and a delivery log. Point it at the address your CRM, shop system or automation tool gives you for incoming webhooks.
What happens if I change a form after visitors have used it?
Renaming or removing a field keeps the answers visitors already sent, and the CSV export uses your current labels. A form that is shown again in a chat, for example after a reload, shows your current version. Turning a form off makes forms already shown in chats say that it is no longer available.
Does a form submission use up messages?
No. Sending a form doesn’t call the AI model, so it doesn’t count toward your monthly messages. Only the agent’s replies count, as usual: one message per reply with a fast model such as Claude Haiku, Gemini Flash or GPT-6 Luna, more with a larger model such as Claude Sonnet, Gemini Pro or Claude Opus.
Are forms shown in every chat?
Forms appear wherever the agent answers on your website: the chat bubble, the inline iframe, the direct link and the Help Page, plus the Playground. They are not shown in temporary chats, because nothing in them is saved; the agent asks the visitor to go back to their saved conversation instead.
